BUCKET CLUB LIABILITY WAIVER AND PHOTO/MEDIA CONSENT (v1 placeholder).
By signing, the parent/guardian or adult member acknowledges the risks of
athletic activity, releases Bucket Club Basketball Development from liability
to the extent permitted by Kentucky law, and records photo/media consent
choices per athlete. This waiver is governed by the laws of the Commonwealth
of Kentucky. For any athlete under 18, this waiver must be signed by that
athlete's parent or legal guardian; a minor cannot sign a legally enforceable
